FH invites you to join an innovative and unique primary health care team located in one of Canada’s fastest growing and culturally diverse cities. The Surrey-Newton Urgent and Primary Care Centre (UPCC) has Full time, Part time and Locum opportunities available.Duties will be split between longitudinal care and urgent care walk in services within the majority of time within the longitudinal stream of care.
Our UPCC opened in the Newton neighbourhood of Surrey, BC in May 2020. We operate in a team-based care model that includes physicians, nurse practitioners, nurses, social workers, clinical counsellors, allied health professionals and administrative support.You will function as an essential member of our clinical care team and manage a panel of patients. We aim to provide patient centred and trauma informed care to our clients and our mandate is to serve the vulnerable and medically complex in the area. Experience in mental health, OAT, and working with at-risk populations will be an asset. Join us to provide timely access to patients, families and caregivers to comprehensive, person-centred, culturally safe and quality care.
