Detailed Overview

Provides and coordinates care to assigned patients in an interdisciplinary Palliative Complex Care Unit setting consistent with current Standards of Professional Practice as outlined by the British Columbia College of Nurses and Midwives (BCCNM) and the established nursing policies, standards, philosophy, vision, purpose and values of Fraser Health.
Responsibilities

  1. Assesses, plans, implements and evaluates nursing care for assigned patients admitted to the designated Palliative Complex Care Unit; utilizes a patient-centered, culturally sensitive approach to evaluate actual or potential problems and/or strengths; plans and performs appropriate interventions and evaluates outcomes in an inter-disciplinary setting.
  2. Collects patient information from a variety of sources using skills of observation, communication and physical assessment to enable the provision of appropriate nursing care; writes timely and accurate reports of relevant observations including patient and family teaching and evaluation of nursing care.
  3. Consults and collaborates with members of the interdisciplinary health care team about patient care; participates in interdisciplinary conferences and partners with patients and families whenever possible to develop an appropriate plan of care.
  4. Acts as an advocate to protect and promote the patient's rights to autonomy, cultural competence, respect, privacy, dignity and access to information.
  5. Participates in quality improvement activities and research by identifying patient care issues and collecting data; provides input into the development, implementation and evaluation of program goals, objectives, policies and procedures.
  6. Participates in the evaluation of patient care programs, techniques, standards and procedures by providing feedback about effectiveness and patient care outcomes.
  7. Participates in the orientation and ongoing education of nursing staff and students by providing information and acting as a preceptor, as appropriate.
  8. Maintains a safe environment for patients, families and staff by adhering to established safety and emergency practices and procedures.
  9. Participates as an active member on nursing and hospital committees and councils for improvement of patient care and/or work environment.
  10. Performs other related duties as assigned.

Qualifications

Education and Experience

Graduation from an approved school of Nursing. One (1) year recent acute medical/surgical nursing or acute palliative care experience, plus completion of a certificate in the designated clinical area, or an equivalent combination of education, training and experience.

Current practicing registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM).

  • Demonstrated knowledge in the field of palliative care.
  • Demonstrated ability to apply clinical knowledge in practice.
  • Demonstrated ability to determine and deal with emergency situations and make appropriate clinical decisions.
  • Ability to communicate effectively, both verbally and in writing.
  • Ability to work independently and as a member of an interdisciplinary team.
  • Demonstrated ability to work with patients/families living with and dying from advanced illness.
  • Demonstrated ability to problem solve and troubleshoot in a variety of situations.
  • Ability to operate related equipment including applicable software applications.
  • Physical ability to carry out the duties of the position.
