JOB SUMMARY

Home Care Services Workers ensure that residents of the Madison live with dignity and are supported in improving their health and wellbeing. In collaboration with the WRHA Home Care Case Coordinator, cases are assigned to Siloam Home Care Services Workers. Siloam Home Care Services workers will collaborate with other Madison team members, as well as external partners including Winnipeg Fire Paramedic Services and the WRHA to provide excellence in care to residents at the Madison.

This position will offer personal care grounded in and guided by the ethics and approaches of Trauma Informed Care, Harm Reduction, and Anti-Oppression.

As part of Siloam Mission's mandate to be a connecting point between the compassionate and less fortunate, the incumbent is expected to facilitate volunteers whenever possible in the performance of his or her duties.

KEY DUTIES & RESPONSIBILITIES

General Duties:

  • Promotes and assists participants in maintaining activities of daily living and select instrumental activities of daily living (ADL and select IADL) and in accordance with Home Care operational directives/guidelines/protocols.
  • Assist residents with performing routine home maintenance, such as laundry and general cleanliness.
  • Organizes work according to participant needs and assigned tasks.
  • Responds to the physical and emotional needs of participants.
  • Provides relevant participant information to the Housing Services team to assist in care planning.
  • With additional education and in accordance with regional policies/operational directives/procedures on assignment and delegation, may be assigned the following participant care tasks:
  • Medication Assistance (oral medications, suppositories, topical medications, transdermal medications, inhaled medications, eye drops/ointments, ear/nose medications, subcutaneous injections).
  • Blood Glucose monitoring Ostomy Care.
  • Adjusting oxygen flow rates.
  • Perform other tasks that may be assigned by Management.

Communication:

  • Responds appropriately to inquiries, relays messages, and initiates appropriate action as directed.
  • Provides written and verbal reports and records participant care provided and changes observed as directed.
  • Establishes and fosters positive interactions and relationships with residents and other members of the Siloam team.
  • Report any issues with resident to management, including the need for supplies, repairs in the room etc.
  • Reports and records critical incidents per Siloam's policies.
  • Submits other required documentation as directed.

Quality Improvement

  • Participates in quality improvement activities as requested.
  • Participates in meetings and teams as assigned.
  • Performs in a manner that reflects the values of trust, compassion, and exceptional service in all interactions with residents, members of the Siloam team and the public.

EDUCATION & EXPERIENCE

  • Successful completion of a recognized Health Care Aide course required.
  • Recent and related patient and/or participant care experience preferred

KNOWLEDGE/SKILLS/BEHAVIORS

  • Excellent organizational, time management and multi-tasking skills required.
  • Information and/or training will be provided within the probation period for the following: Personal Health Information Act (PHIA), Workplace Hazardous Material Information System (WHMIS) and Principles of Routine Practices (Universal Precautions).
  • Regional and Program policies and procedures.
  • Roles and responsibilities of members of the healthcare team.
  • General skills and ability to perform all tasks and responsibilities relevant to the duties of the position.
  • Ability to organize and prioritize assigned workload.
  • Ability to recognize and pursue self-development opportunities.
  • Effective English verbal and written communication skills.
  • Ability to respond to a variety of simultaneous demands.
  • Ability to perform as a team member and seek assistance as necessary.
  • Valid driver's license and vehicle required.
  • Ability to apply to harm reduction approach and housing first principles to day-to-day work.
  • Excellent interpersonal skills and the ability to form relationships with community members including a proven ability to communicate appropriate professional boundaries.
  • Excellent verbal and written communication skills.
  • Knowledge of Indigenous History and Colonial History and its impact on the reality of Indigenous people in Winnipeg.
  • Knowledge of addictions, mental health diagnoses, and co-occurring disorders.
  • Knowledge and ability to adhere to confidentiality standards and legislation.
  • Ability to manage, assess, and work with challenging behaviours.
  • Ability to work independently, under pressure, in a team or independent setting.
  • Strong collaboration skills, both with team members and with external partners.

RELATIONSHIP

  • Daily interaction with residents, staff and volunteers.
  • Attend staff meetings when available.

WORKING CONDITION

  • Position involves extensive physical activity such as lifting, bending.
  • May be exposed to infectious diseases, blood and body fluids, toxic materials, noise, allergens (i.e. residual smoke, pets and or/pet dander), physical and emotional stress.
  • May encounter home environments that could have congested spaces, uneven surfaces (both inside and outside the home environment).
  • Ability to adapt to changes in scheduling on short notice required.
  • This position is subject to TB Immunization or Tuberculin testing as per WRHA policy.
  • Ability to work with individuals who use substances and/or experience mental health issues.
  • Ability to provide support in high intensity incidents including but not limited to medical emergencies and aggressive individuals.
  • Welcome working in environment where Indigenous ceremonies and practices are actively taking place including Smudging.
  • Ability to work with individuals who use substances and/or experience mental health issues.
  • Comfortable conducting participant care in community including encampments, hotels, public housing etc.
  • Comfortable providing transport for community members.
  • Satisfactory Criminal Record Check with Clear Vulnerable Sector Search and a Clear Adult Abuse Registry Check are required. A previous criminal record does not automatically disqualify candidates. We are open to discussing your respective circumstances.
