The BSO Team Lead is a critical role within our communities as it impacts the lives of residents and ignites the warmth of human connection.

As a BSO Team Lead your typical day will impact in the following ways:

  • Providing sound clinical knowledge and skills, acting as clinical expert and resource, and providing comprehensive, quality client/family-centered health care services.
  • Do site visits, as necessary, to assist in decision making for acceptance to the LTCH waitlist.
  • Aiding in the transition of long term care residents with responsive behaviours (new or existing) between community, hospital and neighboring long-term care communities (LTCHs).
  • Reviewing long-term care home applications as they are received by the community and making recommendations as to the communities' ability to meet the client's needs related to responsive/expressive behaviours.
  • Developing a comprehensive transition plan with Care Community BSO partners to support the transition of a new or returning resident into the community.
  • Ensure appropriate screening/ risk assessments are completed for residents with new, worsening or change in behaviours.
  • Coordinate referrals and engage community partners in supporting the resident care needs with responsive behaviours.
  • Facilitate education sessions to build capacity in team members.
  • Supports the management of case reviews, clinical rounds and engage the interprofessional team in the development of an individualized plan of care to meet resident needs.
  • Performing other duties as assigned.

Must haves:

  • Current CPR/First Aid certification.
  • Knowledge of the Principles of Adult Learning and experience educating adults.
  • Minimum (3-5) year' experience working with residents in long term care.
  • Demonstrated knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with dementia, complex mental health, substance use and/or other neurological conditions.
  • All applicants must successfully pass the prescribed Vulnerable Sector Check.
  • As part of legislative requirements, all successful candidates must meet the provincial police check and tuberculosis screening requirements as applicable.

Bonus points for having:

  • Current registration with the appropriate College preferred but not mandatory.
  • Specialized training in Gentle Persuasive Approaches (GPA), U-FIRST, P.I.E.C.E.S., Montessori, DementiAbility are an asset.
