Pay Band: $39.08 - 45.98 per hour

Position Description:


The Ottawa Community Lung Health Program (OCLHP) is a proud program of the Somerset West Community Health Centre (SWCHC). Somerset West Community Health Centre is a well-established, non-profit, multi-service Centre providing health, social and community outreach services with an emphasis on health promotion.

We are looking for a Rehabilitation Specialist to provide Community Based Pulmonary Rehabilitation services to clients as part of the management of their chronic lung disease. OCLHP uses a client-centered approach to care, with a focus on the social determinants of health. As part of the program, the Rehabilitation Specialist will assist clients with chronic lung disease in enhancing their quality of life by improving their mobility, strength, and endurance, and assisting with symptom management with a goal to maintain functional independence.

Requirements


Education:

  • Regulated Healthcare professional currently registered in good standing with their regulatory college in Ontario (Physiotherapist, Respiratory Therapist (with background in Kinesiology), Registered Kinesiologist)
  • Certified Respiratory Educator (preferred), or willingness to obtain certification
  • Current CPR


Key Competencies:

  • Two to five years' experience in pulmonary or cardiac rehabilitation (preferred)
  • Experience working in a community Primary Care setting, an asset
  • Perform exercise assessment, prescription & progression, as well as health education for those with multiple comorbidities, in keeping with the regulations of the appropriate regulatory body.
  • Provide effective care (exercise & education) both in-person & using various communication technologies (e.g. phone, virtual platforms, etc.)
  • Working knowledge of the Chronic Disease Prevention Model and Self-Management programs.
  • Knowledge of primary health care and the social determinants of health
  • Able to work effectively in an inter-professional environment
  • Experience working with disadvantaged communities

  • Strong interpersonal, communication and organizational skills
  • Ability to work independently with minimal supervision
  • Must be comfortable working in a busy environment with multiple demands
  • Proficient use of computers and various software applications
  • Commitment to the principles of health equity, inclusion, and anti-racism/anti-oppression
  • Successful completion of Police record check for vulnerable populations

Language Capability:

  • English is required.
  • French or other language(s) are an asset.
