Job Description

General Responsibilities
1) Ensure care is provided with established nursing standards and principles, physicians' orders and administrative policies, as well as, observe, report and record vital signs, symptoms and conditions of residents / patients. Assure observance of required techniques in nursing care
2) Assist in assessing patients/residents and develop, implement, review and evaluate care plans. Monitor health, notes changes in condition and assess need for referral to doctors; processes and implement physicians' orders

Qualifications
1) Appropriate current certificate of competence from the Approved Institutions for the province
2) Registration or eligible for registration with the Approved Institutions for the province
3) Previous managerial experience in long-term care or community homecare preferred
4) Experience dealing with the cognitively impaired
5) Comprehensive knowledge of nursing and health care practices, as well as Knowledge in Infection Control Practices
6) Demonstrated commitment to working with seniors
7) Ability to present information clearly and effectively both verbally whether in person or over the phone, and in writing; an active listener who projects a friendly demeanor
8) Ability to collect and use information to thoroughly think through problems and situations; puts information from various sources together to see the 'big picture' and generate creative solutions
9) Organizational, planning, time management and multi-tasking skills

10)Utilize RAI-MDS ARD resident list to determine potential residents to admit to program

Role Specific Responsibilities

  • Plan, implement and evaluate both individual and group restorative program based on assessed needs
  • Participate and guide care staff in development and revision of restorative care programs
  • Implementation of program as per Restorative Care Resource Manual.
  • Oversee the completion of resident quarterly assessment and documentation
  • Oversee completion of restorative assessment to determine level of assistance required.
  • Assess resident's abilities, values, interests, past experience and readiness to learn.
  • Candidate must achieve minimum of 85% on the AIS
  • Candidate is results driven with proven ability to empower others to learn
  • Candidate must possess the ability to effectively communicate across disciplines
  • Mentoring and Support of the Interdisciplinary Team as it pertains to Restorative care
  • Proven ability to get things accomplished, organized, good time management skills
  • In-depth knowledge of RAI-MDS
  • Good oral, written communication skills and strong problem-solving abilities
  • Proficient computer skills
  • Looks at the resident from a holistic view point
  • Look at resident issues from more than one disciplines perspective
  • Strengthens the importance and role of each team member in meeting the residents needs
  • RAP's communication tool or RAPs meetings to look at desired outcomes for the resident based on identified issues/strengths
  • Respects and preserves the privacy, confidentiality and security of health care and employee information at all times.
  • In the interest of the safety of residents and family members, employees while on duty are accountable to carry out all duties assigned within the scope of their job in compliance with applicable legislation, standards and company policy.
  • Monitor MDS indicators, outcome scores and RUG scores to inform program planning and evaluation.
  • Coordinates the completion of MDS assessment on residents on Restorative within established timelines.
  • Understands how the MDS assessment integrates with existing clinical tools and provides leadership in streamlining these processes
  • Articulates for staff the strengths of MDS as a clinical assessment tool including how RAP triggers and care plans enhance quality care.
  • Develops and introduces processes, and systems to improve accuracy of RAI content.
  • Facilitates audits of RAI assessments for content accuracy.
  • Develops systems to decrease coding error rates
  • Participates in strategic planning for Restorative Program development/improvement
  • Understands the development of RAP's, quality indicators and RUG case mix products
  • Audit program to ensure goals are clear and measurable, minutes are documented, RAPS addressed in a timely manner, admission and discharge criteria are adhered to, care plan is up-to-date and coding is correct

QUALIFICATIONS

  • Registered Practical Nurse with good standing CNO credentials

  • 3-5 years experience in Long Term Care.

  • Nursing Rehab and Restorative Care Certificate (Will Provide training and certification if necessary)

  • Continuous Quality Improvement Principles, Data Analysis, Microsoft Word, Excel, Outlook

  • Ability to read, write and communicate effectively.

  • Must be flexible with working hours.
