Job Description

Site: West Park Healthcare Centre - 170 Emmett Ave.
Department: Long-Term Ventilation Program
Reports to: Manager, Program Services
Hours: 37.5 hours bi-weekly
Salary: To commensurate with experience and consistent with UHN compensation policy
Status: Permanent, Part-time
Closing Date: February 20, 2025

Position Summary

The primary purpose of this role is to coordinate and facilitate safe and timely transitions of Long-Term Ventilated (LTV) patients from acute care organizations to alternate locations utilizing the resources of the Long-Term Ventilation Strategy in collaboration with external partners and stake holders.

Long-Term Ventilation and Respiratory Complex Care Unit

  • Serve as the coordinating contact between patients, family, team members and referring agencies for LTV/THV.
  • Liaise with the Clinical Manager Provincial LTV to review/update the LTV/THV wait list for West Park.
  • Coordinate timely assessments and admission of appropriate referrals to LTV/THV service and liaise with physicians and the interprofessional team.
  • Co-ordinate the ongoing care of patients from pre-admission to post-discharge while meeting length of stay, occupancy rate and accountability targets.
  • Refer and coordinate consultants / discipline involvement as needed.
  • Support collection of clinical utilization data in collaboration with the Clinical Manager LTV/THV and Respiratory Complex Care.
  • Provide resource clinical leadership for the interprofessional team.
  • Provide leadership in program evaluation, program development and research as appropriate.
  • Participate in internal and external committees.
  • Partner with Clinical Manager LTV/THV to ensure clinical service goals are achieved.
  • Facilitate resolution of clinical care issues / concerns in a timely and effective manner.
  • Support the delivery of person-centred care.
  • Ensure timely and accurate completion of clinical documentation and MDS.
  • Be aware of conditions and measures that may affect the safety of patients, residents, students, staff, volunteers, physicians and visitors including:
  • Compliance with the safety standards of the hospital and applicable legislation
  • Identification and prevention of safety issues and problems
  • Taking corrective action, where possible
  • Attendance at safety meetings/training/updates in accordance with the Centre's standards
  • Ability to work in a manner that exemplifies the core values of West Park: Excellence, Respect, Trust, Collaboration and Accountability
  • Perform other duties as assigned.
  • Demonstrated exceptional clinical assessment skills including the ability to assess clients from pre-admission to post-discharge
  • Knowledge of external agencies and resources
  • Demonstrated ability to partner and work effectively with staff, the interprofessional team, physicians, patients, families and external agencies
  • Demonstrated ability to address complex clinical issues within the context of social determinants of health

Qualifications

Required (Minimum):

  • Bachelor of Science in respective Health Discipline
  • Five years within the practice profession; with LTV experience preferred
  • 3 years of Clinical leadership experience preferred
  • Basic Cardiac Life Support (C Level) Heart Saver Plus Canada (on employment)
  • Excellent interpersonal and communication skills (verbal & written) required
  • Strong customer service focus and excellent problem-solving skills required/Preferred:
  • Member of relevant Regulated Health Discipline professional association of Ontario
  • Minimum of 4 years clinical experience in complex continuing care or acute care setting.
  • Demonstrated exceptional clinical assessment skills including the ability to assess clients from pre-admission to post discharge
  • Knowledge of external agencies and resources
  • Demonstrated ability to address complex clinical issues within the context of social determinants of health
  • Ability to maintain effectiveness in varying environments and with different tasks, responsibilities and people
  • Demonstrated knowledge of and ability to provide person centered care.
  • Demonstrated organizational, problem solving, interpersonal, coaching, negotiation and facilitation skills
  • Strong organizational skills to work independently in an unstructured, demanding environment
  • Demonstrated reliable work habits
  • Ability to work a flexible schedule
  • Strong Windows based computer skills e.g. Microsoft Office Suite
  • Evidence of good performance history and attendance record
