Job Description: The Diabetes/Chronic Disease Educator is responsible for improving the quality of life of persons with chronic disease in the region through the provision of education, treatment and support. The Diabetes/Chronic Disease Educator serves as a resource to health-care professionals and others to ensure consistent, high-quality care for clients with chronic disease by sharing recent advances, knowledge and experiences of care. The Diabetes/Chronic Disease Educator reports to the Manager, Primary Health Care.

Human Resources Exemption: No

Experience

  • Two years of recent nursing experience in a community based setting is required

Education

  • Diabetes Education certificate
  • Bachelor of Science in Nursing (BScN)

Other Education and Training

  • Nursing diploma is acceptable

Knowledge and Abilities

  • Demonstrates effective verbal and written communication skills.
  • Ability to facilitate educational and skill development groups
  • Ability to establish/maintain effective working internal/external relationships
  • Ability to provide public/educational presentations
  • Ability to be innovative in service development, delivery, and problem solving
  • Ability to prioritize, interpret and respond to issues in a timely manner
