Job Description: Reporting to the Manager, the Home Dialysis Nurse Educator integrates clinical expertise and principles of teaching and learning, and chronic disease care, in the provision of chronic kidney disease (CKD) education/training. The recipients of this education/training are home hemodialysis and peritoneal dialysis clients and their families, health care professionals, and students. Teaching occurs in inpatient/outpatient/community settings with individuals/groups. The incumbent is responsible for assessing learning needs and planning, implementing and evaluating educational activities for clients, families, communities, health professionals and students. The Home Dialysis Educator collaborates with the multidisciplinary team members, inpatient/outpatient/hospital departments, community agencies, and vendors/suppliers of dialysis products and equipment to deliver client centred care through educational activities/self-management strategies. The incumbent participates in research/program monitoring/quality improvement initiatives related to home dialysis and CKD management.
