Job Description: Reporting to the manager of Chronic Disease Management the responsibilities include: providing inpatient and outpatient education, clinical and self-management support for patients and families living with cardiovascular disease; working in collaboration with the inter-professional team in acute care and in an outpatient setting ; individual telephone follow-up post discharge; staff education; collaboration with other SHA departments.

Experience

  • Clinical and education experience with cardiac rehabilitation patients/families
  • Experience in teaching adults, facilitation skills, and leading group activities
  • 1 year experience in adult ed/group facilitation with cardio disease mgmt
  • 1 year experience in program/resource development
  • 3 years clinical experience with patients/families living with cardio disease

Education

  • Bachelor of Science in Nursing (BScN)

Licenses

  • Valid Class 5 driver's license

Other Education and Training

  • Basic Life Support Certificate (BLS)
  • Master of Nursing, Adult or Continuing Education is an asset

Competencies

  • Basic - Computer skills
  • Basic - Teaching skills
  • Advanced - Assessment skills
  • Basic - Communication skills
